Issues an HTTPS listener certificate and key pair to a registered PEP.
The derived names are always included and names the caller sends are
added. A refusal is { ok: false, why } carrying its error code. The
reply is the only place the private key ever appears, and the issuance
is audited.
Parameters
Optionalbody: any
name (a PEP registered in this realm), and optionally
dnsNames, ipAddresses, keyAlg and days
Returns Promise<IssueResult>
a promise of the result: on success the certificate, its chain,
fullChainPem, the anchor and privateKeyPem
