- Issues a certificate: the identity rule, the profile, the names from the entry, issuance by the family's CA, and the record kept on the entry.
  Parameters
  - Optionalspec: any
    family, profile, principal, target, publicKeyPem, requested, keySource, privateKeyPem (for a server key), via and replaces
  Returns Promise<any>
  a promise of ok and the issued certificate, or a refusal (audited)
